Child Medical Record: If you already have one on file with us, no need to fill another. You will be able to update information as needed at Orientation. If you have one on file with another care provider, you can bring us a copy of it.
Immunization History: You can substitute an immunization record from your child’s doctor/health department. If your child has not been immunized or is lacking certain immunizations, please complete the appropriate section of the form. If you already have one on file with us, no need to fill another. Health Assessment: This is the form that your child’s physician needs to complete. A doctor’s appointment is not necessarily needed, call your doctor to find out. If you already have one on file with us, no need to fill another. You will be able to update information as needed at Orientation. Authorization for Emergency Medical Care: This form needs to be filled out every school year by every family and signed by a witness.
Parental Permission for Off Premises Trips: This form needs to be filled out every school year by every family.
