There are 4 forms required by KDHE. These forms need to be completed and onsite before a child attends preschool:

– MEDICAL RECORD (you complete)

– IMMUNIZATION HISTORY (you can fill in the form or get a copy from health provider instead)

-HEALTH ASSESSMENT (completed by health provider)

-AUTHORIZATION FOR EMERGENCY MEDICAL CARE (completed by you; we are allowed to be your “Witness” so you have the option to leave that blank)

The first three forms are standard across all licensed childcare settings and preschools so if you have completed these for another program, a copy will work for us. If you attended last year and the information is the same-you do not need to complete these again.

Unfortunately, these forms are not available to fill out online. A few options:  

-edit the attached pdfs and email them back to me

-print them off and complete by hand (take photos to email/text me or deliver to preschool)

-request paper copies to be mailed to you
